Central Health Board of Managers Member Guadalupe Zamora filed these Forms CIS to report a conflict with respect to vendor Integral Care for 2021 and 2022, because he is on the board of trustees for that entity.
The Form CIS Zamora filed for 2019 left the form blank as to the vendor’s name.
Most local governmental organizations require Forms CIS to be filed only when a conflict of interest becomes apparent. Central Health, however, through its Standard Operating Procedure “Duality and Conflict of Interest Procedure” requires members of its Board of Managers to file these on the 20th business day after taking office and at each year’s annual board meeting.
Therefore it appears that a form may be left blank if the manager has no conflict with respect to a vendor.
